Diabetes Self Management Program

Clients of the Diabetes Self-Management Program are guided and encouraged by Memorial's two certified diabetes educators - a registered nurse and a registered dietitian - and a certified diabetes drug therapy management specialist, a staff pharmacist. During the self-management classes, people with diabetes can learn ways to control their disease, instead of letting the disease control them. Classes are offered weekly, by appointment only and a physician order is required. Classes are tailored to client need and can cover Type 1 Diabetes, Type 2 Diabetes or Gestational Diabetes. Class size is also tailored to client need, with one-on-one educational sessions provided as indicated by clinical assessment. Patient numbers for group classes are governed by clinical assessment as well, with follow-up and repeat visits and/or phone support provided to all participants.
