Discharge Planning
Interdisciplinary meetings are held on designated nursing units Monday through Friday from 10 a.m. until noon. Your health care team meets and discusses your discharge plan. Whenever possible your identified needs will be addressed prior to discharge. The patient and family are encouraged to participate in these meetings. A special case conference with the interdisciplinary team can be scheduled upon your request. Ask your nurse to assist you.

When your doctor has discharged you and you are ready to leave, hospital personnel will escort you to an exit. Before leaving, you must be cleared by the Dismissal Office. Your nurse will escort you to the Cashier’s window on the first floor. If your condition prevents you from going to this office, the nurse will take your discharge slip for you.

The Cashier is located on the first floor of the hospital and is open from 8 a.m. to 5 p.m. After 5 p.m., the dismissal slip should be taken to the Emergency Room admission clerk.

Don’t forget to check your room carefully for any personal belongings. Also, remember to pick up any valuables you stored in the hospital security box.

We do not have current contact information on some of our past patients whose records were released. Any of our prior patients may contact Mindy Langford, Director of Medical Records, toll free 888-980-6319, to determine whether your records were released.
