Health Insurance 
- Blue Cross Blue Shield of Texas. Employee contribution required. Benefits effective following a 90-day waiting period from the date of full-time employment.

Dental Insurance - Voluntary employee-paid dental plan available for full-time and part-time employees. The effective date is the first day of the month following a 90-day waiting period from the date of full- or part-time employment.

Group Life Insurance - Employer paid. Benefits effective the first day of the month following a 90-day waiting period from the date of full time employment. Registered/Salaried: benefit amount is two times annual salary, maximum benefit $150,000.00; Non-Registered/Hourly: benefit amount is one time annual salary, maximum benefit $150,000.00.

Accidental Death & Dismemberment - Group Life Insurance. Paid in addition to any benefits payable under life insurance. Maximum benefit $150,000.00.

Spouse and/or Dependent Life - Additional Life Insurance. Employee Paid with Voluntary Election.

Short-Term Disability- Voluntary employee paid policy.  Effective the first day of the month following a 90-day waiting period from the date of full time employment.  Paid benefits begin: 31st day of illness, 16th day of accident.  Benefit amount: Up to 60% of weekly salary.   Maximum Weekly Benefit $1,000.00

PTO (Paid Time Off) - Begin accrual following a 90-day waiting period from the date of full-time employment.

Funeral Leave - A maximum of two eight-hour days are available to a full-time employee for a death in their immediate family. The 90-day probationary period must be completed before eligible for funeral leave.

Long-Term Disability - Texas Hospital Insurance Network - Employer paid. Effective for all full-time employees. Employee is eligible for LTD if, employee is eligible for and is receiving benefits under the Social Security Act.

Retirement - Texas Hospital Insurance Network Employee Contribution/Employer Match. Employees contribute 3-5% of their compensation beginning with full-time employment. Contributions are pre-tax. Vesting is after five years of full-time employment, with 100% employer match.
